Go to the ALiS login page

Visit the Commission's ALiS Licensing System home page and log in with your credentials.

First time using ALiS?

Click below to create your laboratory or entity account before logging in.

Create an account ↗

Select "Laboratory Application for Changes/Updates"

Once logged in, look at the left-hand menu and select Laboratory Application for Changes/Updates.

Fill out and submit your application

Follow the on-screen instructions to complete your application.

Have your form ready. After you click Submit, the system will immediately prompt you to upload your completed Proficiency Monitoring Certification Form. Have it ready before you submit.

Upload your Proficiency Monitoring Certification Form

After clicking Submit, the document upload screen will appear. Upload your completed Proficiency Monitoring Certification Form here. You may also include any other relevant supporting documents in this same section.

Don't navigate away before completing your upload — this screen appears only once per submission.

The Commission will review your submission and follow up if anything additional is needed.

Select "CFE Approval Request" from the menu

In the left-hand menu, select CFE Approval Request.

Note: This option is for CFE Sponsors and Hosts only. If you are not the sponsoring or hosting entity for the course, you do not need to submit this form.

Fill in course information

Follow the on-screen instructions to complete all required fields with your course details.

Have your course agenda ready. You will upload the course information and agenda as part of this submission. Make sure it is finalized before you submit.

Upload your agenda and submit

Once all required fields are complete, upload your course information and agenda, then submit the application to the Commission.

You will receive confirmation from the Commission once your CFE course is approved. Allow adequate lead time before the scheduled course date.
